You are entitled to have a print out of your medical records. We encourage all patients to register with Patient Access to enable online access to your own medical record whenever you require this. This not only saves time but gives you more control over your medical information. Please allow at least 4 weeks for us to process the request.

Please note that we provide a summary of the notes that includes your medical problem list, medications list, allergies and immunisations.
