NHS Cardiovascular Health Check

 

The NHS Health check is a very important check for people aged 40-74. It helps us identify problems like diabetes and cardiovascular disease so we can tackle them early and prevent future complications.
The process involves first having a blood test at one of the local hospitals. When you express interest below, we will create a blood test form and send you a message when it is ready for collection. We will also create a review appt for you with a clinician and you will need to get the blood test done a week before attending the review.

NHS health check

It's great new you want to have the Health check. The first part of the check is to have a blood test that checks your kidneys, cholesterol and blood sugar levels. Please come to the surgery in 7 days to collect the blood form from reception (or when you receive a message advising the form is ready to collect). You will then need to take this form to Kings College Hospital or the Tessa Jowell Hospital to have the blood test done (both are walk in services for blood tests). 

Once you've had the blood test, attend your appointment that will have been booked with our healthcare assistant to complete the check. If you don't have an appointment booked already, these appointments can be booked online using the Patient Access or NHS app.
